At Lotioncrafter, we recognize Vendor Qualification Surveys or Questionnaires are a necessary part of compliance with Good Manufacturing Practices. Based on the sheer quantity of requests we receive and the variable amount of time required to complete these forms, we have created a pricing structure to be able to continue to provide this service.
Pricing is based on the page quantity of each survey, which is indicative of the amount of time it will take to complete the form. While we will do our best to complete these in a timely manner, we cannot guarantee completion by a specific timeline. Please allow several weeks for completion. Forms should be emailed to support@lotioncrafter.com within 5 business days of submitting your order. The Vendor Qualification Documentation request will be canceled for any orders that we have not received surveys for within 5 business days.
